目的 了解山东地区不同人群中SEN病毒 (SENV)感染的状况与其致病性。方法应用逆转录 聚合酶链反应法 (RT PCR)检测了 2 5 4份不同人群和肝病患者血清中SENVDNA ,并对 17例不明原因ALT增高 ,单独SENVDNA阳性的非甲 戊型肝炎患者行肝活检。结果SEN VDNA总阳性检出率为 2 0 .9%( 5 3 / 2 5 4) ,其中非甲 戊型肝炎血清检出率最高为 5 9.6%( 2 8/ 47) ,明显高于健康献血员 3 .2 %( 1/ 3 1)、非肝病患者 5 .5 %( 3 / 5 5 )、输血后丙型肝炎 42 .9%( 12 / 2 8)、血液透析患者 15 .8%( 2 / 13 )和慢性乙肝患者检出率 8.8%( 7/ 80 ) ,SENV在各种人群中存在着显著性差异 (P <0 .0 1)。SENV感染其主要病理学改变为汇管区炎症 ,淋巴细胞集聚 ,碎小坏死 ,小叶内病变较轻 ,呈散在和点状坏死 ,而桥样坏死 ,脂肪性变较少。结论 本研究表明山东地区不同人群及肝病患者中存在着较高SENV感染率 ;SENV感染可能具有嗜肝性 ;而且可能与肝功损害有关 ,是引起非甲 戊型肝炎重要病原 ,输血后丙肝和有血液透析史患者SENV感染率较高 ,提示对献血员还应严格筛选
Objective To understand the status and pathogenicity of SENV infection among different populations in Shandong Province. Methods The serum levels of SENVDNA were detected by reverse transcription polymerase chain reaction (RT-PCR) in 245 individuals with liver disease and 17 patients with unexplained ALT. Liver biopsy was performed in patients with SENVNA-positive non-E hepatitis. . Results The overall positive detection rate of SEN VDNA was 20.9% (5 3/2 45). The highest detection rate of non-A hepatitis E was 5 9.6% (28/47), which was significantly higher than that of healthy blood donors 3.2% (1/3 1) of the patients, 5.5% (3/5 5) of non-liver diseases, 42.9% (12/28) of hepatitis C after transfusion and 15.8% 2/13), and chronic hepatitis B (8.8%). The prevalence of SENV was significantly different among all groups (P <0.01). SENV infection of the main pathological changes for the portal area inflammation, lymphocyte aggregation, broken small necrosis, lobular lesions were mild, scattered and punctate necrosis, and bridge-like necrosis, less fatty changes. Conclusions This study shows that there is a high infection rate of SENV among different populations and patients with liver disease in Shandong Province. SENV infection may have hepatomegaly; and may be related to liver damage, which is an important pathogen causing non-A hepatitis and hepatitis C Patients with a history of hemodialysis SENV infection rate is higher, suggesting that blood donors should also be strictly screened
