目的:分析活动性类风湿性关节炎(RA)患者血小板相关参数和急性时相反应物水平的变化及三藤通痹丸对其的干预作用。方法:将符合入选标准的107例活动期RA患者,按区组随机化原则随机分为三藤通痹丸组(治疗组)53例,正清风痛宁组(对照组)54例。采用ESR自动分析仪和特定蛋白分析仪常规检测ESR、CRP的水平;采用全自动血细胞分析仪检测血小板相关参数,进行前后对比治疗研究。结果:活动性RA患者血小板相关参数PLT、PCT异常,急性时相反应物水平明显升高,与健康人群比较差异有显著性(P<0.05或P<0.01)。经治疗后,两组患者上述各观察指标有不同的回落,与治疗前比较有显著性差异(P<0.05或P<0.01);治疗后治疗组与对照组比较,差异有显著性(P<0.05或P<0.01)。结论:RA患者病情活动时,血小板参数PLT、PCT显著升高,急性时相反应物释放增多,血小板参数PLT、PCT与ESR、CRP一样,可作为反映RA病情活动性的一项客观指标;三藤通痹丸对RA患者的愈后影响与抑制急性时相反应物的释放,降低血小板参数,减弱致炎因子的刺激,消除或减弱炎症和免疫反应有关。
OBJECTIVE: To analyze the changes of platelet-related parameters and acute phase reactants in patients with active rheumatoid arthritis (RA) and the effect of Sutong Tongbi Pill on it. Methods: A total of 107 patients with active RA who meet the inclusion criteria were randomly divided into three groups according to the principle of randomization: 53 cases of treatment group and 54 cases of Zhengqing Fengtongning group (control group). The levels of ESR and CRP were detected routinely by ESR automatic analyzer and specific protein analyzer. Platelet-related parameters were detected by automatic hematology analyzer and compared before and after treatment. Results: The PLT, PCT abnormalities and acute phase reactant levels in active RA patients were significantly higher than those in healthy subjects (P <0.05 or P <0.01). After treatment, the two groups of patients with the above indexes have different fall, compared with before treatment were significantly different (P <0.05 or P <0.01); after treatment, the treatment group compared with the control group, the difference was significant (P < 0.05 or P <0.01). Conclusions: Platelet parameters PLT and PCT are significantly increased, the release of acute phase reactants is increased, and platelet parameters PLT, PCT and ESR and CRP are the same, which can be used as an objective index to reflect the activity of RA. The effect of Toutongbi Pill on the later patients with RA is related to inhibiting the release of acute phase reactants, lowering the platelet parameters, weakening the stimulation of inflammatory factors and eliminating or attenuating inflammation and immune response.
