In most patients with digoxin and quinidine, plasma digoxin levels increase by 100-150%, but the rate of increase varies from patient to patient. When digoxin is used in combination with quinidine, digoxin renal clearance is reduced by 25 to 40%, but the reduction does not appear to be due to a decrease in glomerular filtration rate. Quinidine can reduce the total digoxin total clearance rate of 33 to 64%. Quinidine and digoxin use the latter to increase the effectiveness of 15%, and some studies have shown that quinidine can promote the absorption of digoxin. Animal experiments show that quinidine can inhibit the active transfer of digoxin by serosal mucosa. According to the above mechanism, digoxin secreted with bile is inhibited and digoxin enterohepatic circulation is interrupted, thereby increasing its absorption or decreasing its intestinal elimination.
