目的研究尿毒症继发甲状旁腺功能亢进(甲旁亢)对心脏结构的影响。方法选取45例中日友好医院接受甲状旁腺切除术(PTX)手术治疗的尿毒症继发甲旁亢患者,为组1;选取60例台湾高雄长庚医院进行腹膜透析的尿毒症继发甲旁亢患者,为组2。对两组的各项资料进行比较,分析尿毒症继发甲旁亢对心脏结构的影响。结果两组患者例数及平均年龄比较,差异无统计学意义(P>0.05);两组患者平均透析龄、平均甲状旁腺激素(i PTH)、血钙、血磷和Ca-P乘积比较,差异均有统计学意义(P<0.05);两组患者左心房直径(LA)、射血分数(EF)比较,差异有统计学意义(P<0.05)。结论尿毒症继发甲旁亢对患者左室射血分数和左心房直径的影响可造成对心脏结构的影响。i PTH升高与透析龄、血钙、血磷和Ca-P乘积呈正相关;对左心室收缩末期容积、舒张末期容积、主动脉根直径、室间隔厚度无明显影响。
Objective To study the effect of hyperparathyroidism (hyperparathyroidism) secondary to uremia on cardiac structure. Methods Forty-five cases of secondary hyperparathyroidism of uremia treated by parathyroidectomy (PTX) in 45 Chinese-Japanese Friendship Hospitals were selected as group 1. Sixty cases of peritoneal dialysis in Kaohsiung-Chang Gung Memorial Hospital were enrolled in this study. Kang patients, for the group 2. The data of two groups were compared to analyze the influence of hyperparathyroidism on cardiac structure secondary to uremia. Results There was no significant difference between the two groups in the number of patients and the average age (P> 0.05). The mean dialysis age, mean PTH, serum calcium, phosphorus and Ca-P in two groups were compared (P <0.05). There was a significant difference in left atrial diameter (LA) and ejection fraction (EF) between the two groups (P <0.05). Conclusion The effect of hyperparathyroidism secondary to uremia on left ventricular ejection fraction and left atrial diameter in patients may have an impact on cardiac structure. i PTH increased with dialysis age, serum calcium, phosphorus and Ca-P product was positively correlated; the left ventricular end-systolic volume, end-diastolic volume, aortic root diameter, ventricular septal thickness had no significant effect.
