目的 :检测膀胱肿瘤患者尿脱落细胞端粒酶活性表达 ,探讨端粒酶活性对膀胱肿瘤早期诊断及术后监测的意义。方法 :采用基于PCR的端粒重复序列扩增 (TRAP)结合银染的方法 ,分别对自 1998年 1月至 1999年 6月收集的 2 5例膀胱肿瘤组织标本及尿脱落细胞标本 ,和 2 3例非膀胱肿瘤患者尿脱落细胞标本进行端粒酶活性检测。结果 :2 5例膀胱肿瘤组织及相应患者的自排脱落细胞端粒酶活性阳性率分别为 84%和 80 % ,而 2 3例非膀胱肿瘤患者尿脱落细胞仅一例表达有端粒酶活性 ,膀胱肿瘤患者与非肿瘤患者自排尿脱落细胞端粒酶活性表达比较差异有显著性 (P <0 .0 1) ,G1,G2 ,G3期膀胱肿瘤患者自排尿脱落细胞端粒酶活性阳性率分别为 71% ,70 % ,10 0 % ,G3期阳性率明显高于G1,G2期。结论 :膀胱肿瘤尿脱落细胞端粒酶活性检测可作为一种无侵入性检查用于膀胱肿瘤的早期筛选诊断和术后复发的监测
Objective: To detect the expression of telomerase activity in urine exfoliated cells in patients with bladder cancer, and to explore the significance of telomerase activity in the early diagnosis and postoperative monitoring of bladder cancer. Methods: Twenty-five specimens of bladder tumor and urine exfoliated cells collected from January 1998 to June 1999 were collected by PCR-based TRAP combined with silver staining. Urinary exfoliated cells from 3 non-bladder cancer patients were tested for telomerase activity. Results: The positive rate of telomerase activity was 84% and 80% respectively in 25 cases of bladder tumor and corresponding patients, while only one case of exfoliated cells in 23 cases of non-bladder tumor expressed telomerase activity, The expression of telomerase activity in exfoliated cells from bladder cancer patients and non-tumor patients was significantly different (P <0.01). The positive rates of telomerase activity in exfoliated cells from patients with bladder tumors in G1, G2 and G3 were 71%, 70%, 100%, the positive rate of G3 was significantly higher than that of G1 and G2. Conclusion: The detection of telomerase activity in exfoliated urothelial cells of bladder tumor can be used as a noninvasive test for early screening and postoperative recurrence of bladder cancer
