目的探讨超敏C反应蛋白(hs-CRP)水平、D-二聚体(D-Dimer)和纤维蛋白原(FIB)在高血压内皮损伤中的临床应用价值。方法 86例高血压患者,按有无血管并发症分为重症组(41例)和轻症组(45例),另选取40例健康体检者进行对照作为无症组。分别检测三组人员的hs-CRP、D-Dimer和FIB水平,并进行比较分析。结果高血压患者的hs-CRP、D-Dimer和FIB水平与阳性率均明显高于无症组,差异均具有统计学意义(P<0.05);重症组患者hs-CRP为(9.56±3.15)mg/L、D-Dimer为(1.08±0.93)mg/L、FIB为(5.90±2.90)g/L,均明显高于轻症组的(5.21±2.30)mg/L、(0.72±0.53)mg/L、(4.80±1.90)g/L,差异均具有统计学意义(P<0.05)。且重症组上述指标阳性率分别为78.0%、70.7%、73.2%,高于对照组的57.8%、51.1%、53.3%,差异均具有统计学意义(P<0.05);经过1周合理治疗,重症组hs-CRP、D-Dimer和FIB水平三项指标均明显下降,差异具有统计学意义(P<0.05)。结论 hsCRP、D-Dimer和FIB检测能够有效地反映高血压的进程及患者血管内皮损伤的程度。
Objective To investigate the clinical value of hs-CRP, D-dimer and fibrinogen (FIB) in hypertensive endothelial injury. Methods Totally 86 hypertensive patients were divided into severe group (41 cases) and mild group (45 cases) without vascular complications. Another 40 healthy subjects were selected as the asymptomatic group. Three groups of hs-CRP, D-Dimer and FIB levels were detected and compared. Results The levels of hs-CRP, D-Dimer and FIB in patients with hypertension were significantly higher than those in asymptomatic patients (P <0.05). The hs-CRP in severe patients was (9.56 ± 3.15) (1.08 ± 0.93) mg / L for D-Dimer and (5.90 ± 2.90) g / L for FIB were significantly higher than that of mild group (5.21 ± 2.30) mg / L and (0.72 ± 0.53) mg / L and (4.80 ± 1.90) g / L, respectively, with statistical significance (P <0.05). The positive rates of the above indexes in the severe group were 78.0%, 70.7% and 73.2%, respectively, which were higher than those in the control group (57.8%, 51.1%, 53.3%, P <0.05) Severe group of hs-CRP, D-Dimer and FIB levels were significantly decreased three indicators, the difference was statistically significant (P <0.05). Conclusion The detection of hsCRP, D-Dimer and FIB can effectively reflect the progression of hypertension and the degree of vascular endothelial injury in patients.
