目的:探讨全身炎症反应综合征(SIRs)患者血浆胃动素(MTL)的变化及其临床意义。方法:符合SIRS诊断48例,分别于诊断SIRS后1、8 h和第2、3、4 d清晨空腹抽肘静脉血3 ml,测定MTL和炎性细胞因子肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)。并设对照组。结果:SIRS患者MIL水平升高,第2 d为最高,各监测点持续增高,与对照组比较差异有显著性(P<0.01);并与炎性细胞因子同步升高,呈显著相关性(P<0.01);伴有应激性溃疡出血的病人MTL水平更高,与不伴有出血的病人比较差异明显(P<0.01)。结论:SIRS患者MTL水平的升高参与了胃肠黏膜损伤、应激性溃疡、肠道细菌和毒素移位等重要的病理生理过程。
Objective: To investigate the changes of plasma motilin (MTL) in patients with systemic inflammatory response syndrome (SIRs) and its clinical significance. Methods: Forty-eight patients were diagnosed as having SIRS and 3 ml of elbow venous blood were collected at 1, 8, and 2, 3, 4 days after SIRS respectively. The levels of MTL and TNF-α ), Interleukin-6 (IL-6). And set the control group. Results: The levels of MIL in SIRS patients were elevated at day 2, the highest at day 2, and all the monitoring sites continued to increase. There was a significant difference between the two groups (P <0.01). The level of MIL was also significantly increased with the increase of inflammatory cytokines P <0.01). Patients with stress ulcer hemorrhage had higher MTL levels than those without hemorrhage (P <0.01). CONCLUSIONS: Elevated MTL level in SIRS patients is involved in important pathophysiological processes such as gastrointestinal mucosal injury, stress ulcer, intestinal bacteria and toxin translocation.
