目的 探讨抗氧化剂联合接触镜治疗圆锥角膜急性水肿期的治疗效果。方法 16例 (18只眼 )采用抗氧化剂 :2 %谷胱甘肽滴眼液局部点眼。口服维生素 C、维生素 E。为减轻水肿及炎症反应 ,局部加点 4%氯化钠滴眼液 ,0 .1%双氯灭痛滴眼液。防止合并感染用 0 .3%环丙沙星滴眼液局部点眼。经药物治疗水肿减轻 ,能耐受接触镜 ,可戴接触镜进行光学矫正。分别选戴 RGPCL 16只眼 ,Piggyback lens2只眼 ,定期复查。结果 18只眼圆锥角膜急性水肿期经治疗后 3~ 5周水肿基本消退。瘢痕形成 ,呈半透明 ,角膜锥顶逐渐扁平化。偏离视轴区瘢痕矫正视力良好 ,视力矫正结果 0 .5~ 0 .8者占 11只眼 (其中 0 .8以上 4只眼 )占 6 1%。 0 .5以下~ 0 .1者 5只眼。 0 .0 5者 2只眼 ,占 11%。经观察全部病例 ,无并发症 ,角膜散光及近视度逐渐减少 ,角膜曲率半径逐渐增大 ,角膜地形图显示SRI、SAI值逐渐减少 ,逐渐向角膜表面规则和球面性转化。结论 谷胱甘肽、维生素 C、E均属于抗氧化药物。对于圆锥角膜急性水肿期减轻水肿 ,愈合后弹力层裂孔起到有效的积极治疗作用。接触镜有较好的矫形作用 ,对控制病情发展有一定的治疗作用。此方法具有见效快 ,疗效可靠 ,容易操作 ,具有安全性高的优越性
