目的了解在神经肌电图检测中不同刺激点对急性腕下垂病人神经病变定位诊断的价值。方法对42例急性手腕下垂患者进行上肢桡神经肌电图检测。结果(1)用针极肌电图检测,患侧未记录到运动单位电位;(2)用神经肌电图测得,第一组刺激点诱发出的电位时限,末端潜伏期、运动神经传导速度,两侧上肢均在正常范围,对比无差异(P>0.05);第二组刺激点测得的神经肌电图电位变化表现为,患侧电位时限增宽、末端潜伏期延长(P<0.01)、运动传导速度明显减慢(P<0.01)。结论神经肌电图对急性手腕下垂的早期诊断是一种较有效的检测方法,检测刺激点以第二组为佳。
Objective To investigate the value of different stimulation points in neuromuscular examination for the localization of neuropathies in patients with acute carpal ptosis. Methods Twenty-two patients with acute wrist drop were examined by radial nerve EMG. Results (1) With the needle electromyography, the motor unit potential was not recorded in the ipsilateral. (2) The time, terminal latency, motor nerve conduction velocity (P> 0.05). The changes of electromyogram potentials in the second stimulation group were as follows: the time limit of the affected side widened and the terminal latency was prolonged (P < 0.01), the speed of motor conduction slowed down significantly (P <0.01). Conclusion Electromyography is an effective method for the early diagnosis of acute wrist sag. The second group is the best to detect the stimulus points.
