[目的]分析颈部转移性淋巴结超声造影时间—强度曲线特点,以探讨超声造影对诊断颈部转移性淋巴结的价值。[方法]对75例颈部淋巴结病变的患者进行超声造影检查,共75枚淋巴结(反应性增生34枚,转移性41枚)。整个淋巴结设为感兴趣区,如有坏死液化及范围较大的钙化区,则剔除此无血供区。对获得的时间—强度曲线参数(基础强度、峰值强度、上升支斜率、始增时间、达峰时间、上升时间、峰值浓度减半时间、平均渡越时间)进行比较分析。[结果]峰值强度:转移性淋巴结(51.8181±6.2808)d B,反应性增生淋巴结(58.0895±5.7174)d B,P<0.01;上升支斜率:转移性淋巴结1.8266±0.6738,反应性增生淋巴结0.9451±0.6639,P<0.01;上升时间:转移性淋巴结(5.5057±1.2348)s,反应性增生淋巴结(6.3841±2.1820)s,P=0.032。转移性淋巴结峰值强度低于反应性增生淋巴结,上升时间短于后者,上升支斜率小于后者,差异均有统计学意义(P<0.05),而二者间的始增时间、达峰时间、峰值浓度减半时间及平均渡越时间等的差异无统计学意义。[结论]转移性淋巴结与反应性增生淋巴结在超声造影时间—强度曲线中的差异可为鉴别诊断提供更多参考信息,对临床诊断具有指导意义。
[Objective] To analyze the time-intensity curve of neck metastatic lymph node contrast-enhanced ultrasound (CEUS) in order to investigate the value of contrast-enhanced ultrasound in the diagnosis of metastatic lymph nodes in the neck. [Method] Totally 75 lymph nodes (34 reactive hyperplasia and 41 metastatic) were examined in 75 cases of cervical lymphadenopathy. The entire lymph node is set as the region of interest. If there is necrosis liquefaction and a large area of calcification, the blood-free zone is excluded. The obtained time-intensity curve parameters (basic intensity, peak intensity, rising branch slope, initial rise time, peak time, rise time, peak concentration half-time, average transit time) were compared and analyzed. [Results] Peak intensity: metastatic lymph node (51.8181 ± 6.2808) d B, reactive hyperplastic lymph node (58.0895 ± 5.7174) d B, P <0.01; rising branch slope: metastatic lymph node 1.8266 ± 0.6738, reactive hyperplastic lymph node 0.9451 ± 0.6639, P <0.01; Rise time: metastatic lymph nodes (5.5057 ± 1.2348) s, reactive hyperplastic lymph nodes (6.3841 ± 2.1820) s, P = 0.032. The peak value of metastatic lymph nodes was lower than that of reactive hyperplastic lymph nodes, and the rise time was shorter than the latter. The slope of rising branch was smaller than that of the latter (P <0.05), while the initial increment time, peak time , The peak concentration of half the time and the average transit time of the difference was not statistically significant. [Conclusions] The difference between time-intensity curves of metastatic lymph nodes and reactive hyperplastic lymph nodes in contrast-enhanced ultrasound can provide more reference information for differential diagnosis, which is instructive for clinical diagnosis.
