高强度聚焦超声波是近年来应用于青光眼治疗的一种新方法。作用机理是利用适当控制能量的聚焦超声波(强度10 kW/cm~2,频率4.6 MHz,透射时间5秒),在治疗区巩膜组织内产生灼热作用而造成组织破坏。适用于各种顽固性青光眼的治疗和重建滤过术后早期失效的滤过泡。其降低眼压的机理,可能是治疗区巩膜变薄使房水外引流增加,睫状体上皮的破坏使房水生成减少和房水向脉络膜上腔引流增加。治疗效果优于睫状体冷冻术。主要并发症有术后早期眼压升高、葡萄膜炎、巩膜变薄和低眼压。这种新的治疗方法尚待进一步完善。
