目的 探讨重复经颅磁刺激对缓解期双相抑郁障碍患者认知功能的影响.方法 将80例缓解期双相抑郁障碍患者随机分为研究组与对照组,每组40例.研究组给予重复经颅磁刺激,对照组给予无效刺激,每周5次,观察4周.分别于治疗前后采用威斯康星卡片分类测试及事件相关电位P300测试评定两组患者的认知功能.比较两组不良反应发生率.结果 治疗后研究组威斯康星卡片分类测试总应答数、完成分类数及正确应答数均显著升高(P＜0.01),持续性错误数及错误应答数均显著降低(P＜0.01);对照组与治疗前比较差异无无统计学意义(P＞0.05).治疗后研究组P300潜伏期显著短于治疗前(P＜0.01),波幅显著长于治疗前(P＜0.01);对照组与治疗前比较差异无统计学意义(P＞0.05).两组不良反应发生率比较差异无统计学意义(P＞0.05).结论 重复经颅磁刺激可以显著改善双相抑郁障碍缓解期患者的认知功能,值得推广应用.“,”Objective To investigate the effects of repetitive transcranial magnetic stimulation on cognitive function in patients with bipolar depressive disorder during the remission stage. Methods 80 cases of patients with bipolar depression were randomly divided into the observation group and the control group, 40 cases in each group. The observation group was given rTMS, and the control group was given ineffective stimulus,5 times one week and patients were observed for 4 weeks. The WCST and event-related potential P300 were used to evaluate the cognitive function of the two groups before and after treatment, then rate of adverse reaction of the two groups were compared. Results The total number of responses, complete classification number, number of correct response in the observation group after treatment were significantly increased (P＜0.01),persistent error number and number of correct response were significantly declined (P＜0.01); in the control group, the difference was not statistic significant compared with that before treatment (P＞0.05). P300 peak latency in the observation group after treatment was significantly shorter than that before treatment (P＜0.01), amplitude was significantly longer than that before treatment (P＜0.01); in the control group, the difference was not statistic significant compared with that before treatment (P＞0.05). The difference of adverse reaction rate between the two groups had no statistic significance (P＞0.05). Conclusion rTMS can significantly improve cognitive function in patients with bipolar depressive disorder during the remission stage, it is worthy of popularization and application.
