在结肠、直肠外科中,吻合口裂开是个难题。尽管提高血运和阻止局部缺血坏死是吻合成功的先决条件,但在实际工作中,在血供不足的情况下,被迫行吻合术而致手术失败。迄今,仍无防止局部缺血坏死的满意药物。已经证实,前列腺素,特别是前列环素PGI_2是有效血管扩张剂和具有细胞保护作用,在防止局部缺血坏死中有效果。然而,由于不稳定和产生低血压等副作用,限制了临床应用。最近,找到了一种新的、稳定的前列环素类似物DP-41483,在狗缺血性结肠吻合术中,证实了该药的保护作用。
In colon, rectal surgery, anastomotic dehiscence is a problem. Although increasing blood supply and stopping ischaemia and necrosis are prerequisites for success, in actual work, when blood is scarce, they are forced to perform anastomosis and the operation fails. To date, there are no satisfactory drugs to prevent ischemic necrosis. It has been shown that prostaglandins, especially prostacyclin PGI_2, are potent vasodilators and have cytoprotective effects that are effective in preventing ischemic necrosis. However, clinical application is limited due to the side effects such as instability and hypotension. Recently, a new and stable prostacyclin analog, DP-41483, has been found to demonstrate its protective effect in dog ischemic colorectal anastomosis.
