探讨肿瘤坏死因子(TNF)、内皮素(ET)、降钙素基因相关肽(CGRP)在急性颅脑损伤的临床意义。方法应用放免法动态测定41例急性颅脑损伤患者和31例正常对照组脑脊液TNF、ET、CGRP含量,以观察脑脊液TNF、ET、CGRP水平在颅脑损伤组和对照组中的变化。结果颅脑损伤患者急性期脑脊液TNF、FT含量显著升高,CGRP含量显著降低,且与伤情轻重程度明显相关;TNP与ET呈正相关,TNF、ET与CGRP呈负相关。结论脑脊液TNF、ET水平增高和CGRP水平降低可能是急性颅脑损伤继发性病理生理损害的重要因素之一;脑脊液TNF、ET、CGRP动态含量,可能是判断其预后的客观指标。
To investigate the clinical significance of tumor necrosis factor (TNF), endothelin (ET) and calcitonin gene related peptide (CGRP) in acute craniocerebral injury. Methods The contents of TNF, ET and CGRP in cerebrospinal fluid of 41 acute craniocerebral injury patients and 31 normal controls were measured by radioimmunoassay to observe the changes of cerebrospinal fluid TNF, ET and CGRP levels in the brain injury group and the control group. Results The levels of TNF and FT in cerebrospinal fluid of patients with craniocerebral injury were significantly increased, while the levels of CGRP were significantly decreased. The levels of TNF and FT were positively correlated with the severity of injury. The levels of TNF, ET and CGRP were negatively correlated. Conclusions Increased levels of TNF and ET and decreased levels of CGRP in cerebrospinal fluid may be one of the important factors of secondary pathophysiologic damage in acute craniocerebral injury. The dynamic levels of TNF, ET and CGRP in cerebrospinal fluid may be objective indexes to judge their prognosis.
