用形态计量学方法对41例刮宫葡萄胎、35例首次刮宫的侵袭性葡萄胎和42例切除子宫的侵袭性葡萄胎进行了细胞核形态的定量研究。结果表明,3组葡萄胎细胞滋养细胞核的平均面积、平均周长、平均比表面及平均面积等效直径差异均有显著性(P<0.01~0.001);合体滋养细胞核各项形态学参数差异不明显。判别分析:首次刮宫葡萄胎组织良恶性诊断的符合率为84.5%。提示葡萄胎细胞滋养细胞核的形态学参数可作为区别葡萄胎良恶性的指标。与图像分析及传统病理组织分级方法比较表明,手工测试与图象分析同样可信,比传统病理组织分级方法更具有客观性和诊断价值。
Forty-one cases of curettage hydatidiform mole, 35 cases of first curettage of invasive hydatidiform mole and 42 cases of excised uterine invasive hydatidiform mole were studied by morphometric methods. The results showed that there were significant differences in the average area, average circumference, average specific surface area and average area equivalent diameter of three hydatid cysts (P <0.01 ~ 0.001). The morphological parameters of syncytiotrophoblasts were not different obvious. Discriminant analysis: the first curettage mole diagnosis of benign and malignant lesions was 84.5%. It is suggested that morphological parameters of hydatid cyst trophoblastic nuclei may be used as an index to distinguish between malignant and malignant hydatidiform mole. Compared with the image analysis and traditional histopathological grading methods, manual testing is as credible as image analysis and more objective and diagnostic than traditional pathological grading methods.
