目的:探讨骨肉瘤组织中O(6)-甲基鸟嘌呤-DNA甲基转移酶[O(6)-methylguanine-DNA methyltransferase,MGMT]的表达与顺铂(cisplatin,DDP)治疗后瘤组织坏死程度的关系。方法:选取2001年1月至2008年4月北京307医院骨科收治的骨肉瘤患者76例,全部患者均以标准的DDP方案治疗3个疗程以上,然后手术切除肿瘤。以免疫组化法检测顺铂治疗前活检标本中MGMT的表达,以H-E染色观察手术切除骨肉瘤组织的坏死程度,分析MGMT表达和肿瘤坏死程度的关系。结果:本组患者骨肉瘤组织中MGMT蛋白的表达率为68%(52/76),其毛细胞血管扩张型、成骨细胞型、成软骨细胞型、成纤维细胞瘤型间的MGMT表达率无差异。MGMT阴性的肿瘤经DDP化疗后的组织坏死率高,反之肿瘤坏死率低(P<0.01)。结论:MGMT蛋白表达与DDP化疗后骨肉瘤组织的坏死程度呈负相关,该指标部分地反映肿瘤的耐药性和DDP的疗效。
OBJECTIVE: To investigate the expression of O (6) -methylguanine-DNA methyltransferase (MGMT) in osteosarcoma tissue and the effect of cisplatin on the tumor necrosis Degree of relationship. Methods: From January 2001 to April 2008, 307 patients with osteosarcoma from Beijing Orthopedic Hospital were enrolled in this study. All of the patients were treated with standard DDP regimen for more than 3 courses of treatment, and the tumors were surgically removed. The expression of MGMT in biopsy specimens before treatment with cisplatin was detected by immunohistochemistry. The necrosis degree of osteosarcoma was observed by H-E staining. The relationship between the expression of MGMT and the degree of tumor necrosis was analyzed. Results: The expression rate of MGMT protein in osteosarcoma tissue was 68% (52/76) in this group. The expression rate of MGMT in vasodilative, osteoblastic, chondroblastic and fibroblastoid types No difference. Tumor necrosis rates of MGMT-negative tumors after DDP chemotherapy were high, whereas tumor necrosis rates were low (P <0.01). Conclusion: The expression of MGMT protein is negatively correlated with the degree of necrosis of osteosarcoma tissue after DDP chemotherapy. This index partly reflects the drug resistance of tumor and the efficacy of DDP.
