目的探讨慢性乙醇中毒性周围神经病(CAPN)的电生理改变特点。方法回顾分析2005~2008年在本院神经内科就诊的24例CAPN患者及26例对照组的电生理改变特点,所有对象均检测双侧正中神经、尺神经、胫后神经、腓神经的神经传导速度(NCV),并在腘窝引出H反射。结果CAPN患者电生理均有不同程度改变,CAPN组的NCV及H反射异常率明显高于对照组,两组相比差异有统计学意义(P<0.05);下肢异常率(83.3%)高于上肢异常率(76.5%);SCV异常率(84.7%)高于MCV异常率(75.5%)。结论NCV及H反射对乙醇中毒性周围神经病的确诊有帮助,戒酒并辅以B族维生素治疗有效。
Objective To investigate the electrophysiological changes of chronic alcoholic toxic peripheral neuropathy (CAPN). Methods The electrophysiologic characteristics of 24 patients with CAPN and 26 control subjects who were treated in our hospital from 2005 to 2008 were retrospectively analyzed. All the subjects were examined for nerve conduction in bilateral median nerve, ulnar nerve, tibial nerve and peroneal nerve Speed (NCV), and leads to H reflection in the popliteal fossa. Results The electrophysiological changes of patients with CAPN varied to some extent. The abnormalities of NCV and H in CAPN group were significantly higher than those in control group (P <0.05). The abnormal rate of lower limbs was higher than that in control group (83.3%) The abnormal rate of upper limbs was 76.5%. The abnormal rate of SCV was higher than that of MCV (84.7%) (75.5%). Conclusions NCV and H reflexes are helpful in the diagnosis of alcohol-induced peripheral neuropathy. Alcohol abstinence and supplementation with B vitamins are effective.
