目的:讨论心肌酶谱和心肌肌钙蛋白Ⅰ(cardiac troponin Ⅰ,cTnⅠ)水平检测对不同程度的新生儿缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)患儿的诊断意义。方法:选取本院收治的50 HIE例患儿,按照其病情的轻重区别分为轻度组(n=23)、中度组(n=18)和重度组(n=9),并选择同一时期我院出生的正常新生儿20名。分析对比其心肌酶谱及cTnⅠ水平变化情况。结果:HIE患儿心肌酶谱内CK、CK-MB、LDH、HBDH均高于正常新生儿,组间相比具有统计学意义(P<0.05)。不同程度的HIE新生患儿cTnⅠ随着病情的加重而升高,差异具有统计学意义(P<0.05)。结论:血清心肌酶谱及cTnⅠ的检测对新生儿HIE的诊断和治疗具有重要的临床意义和价值。
Objective: To investigate the significance of myocardial enzymes and cardiac troponin Ⅰ (cTnⅠ) levels in the diagnosis of neonates with hypoxic-ischemic encephalopathy (HIE). Methods: 50 HIE patients admitted to our hospital were divided into mild group (n = 23), moderate group (n = 18) and severe group (n = 9) according to their severity. 20 normal newborns born in our hospital during the period. Analysis and comparison of myocardial enzymes and cTn Ⅰ level changes. Results: The levels of CK, CK-MB, LDH and HBDH in myocardial enzymes of HIE children were significantly higher than those of normal newborns (P <0.05). The levels of cTnI in neonates with HIE increased with the severity of the disease, the difference was statistically significant (P <0.05). Conclusion: Serum myocardial enzymes and cTn Ⅰ detection of neonatal HIE diagnosis and treatment has important clinical significance and value.
