目的探讨经阴道子宫输卵管四维超声造影(4D-Hy Co Sy)在诊断输卵管通畅性中的应用价值。方法选取2014年11月至2015年12月在辽宁省沈阳市妇婴医院行4D-Hy Co Sy的患者40例,以腹腔镜检查结果为金标准,评价患者的输卵管通畅性以及分析4D-Hy Co S诊断能力。结果本研究中40例患者共80条输卵管,4D-Hy Co Sy与腹腔镜检测结果差异无统计学意义(Kappa=0.0769,P=0.803);4D-Hy Co Sy检测输卵管是否阻塞的灵敏度为92.0%(46/50),特异度为90.0%(27/30),正确率为91.2%(73/80)。结论 4D-Hy Co Sy能够立体观察子宫、输卵管形态及粘连情况,且诊断输卵管通畅性的灵敏度、特异度及正确率均较高,在一定程度上能够代替腹腔镜。
Objective To investigate the value of transvaginal four-dimensional contrast-enhanced ultrasound (4D-Hy Co Sy) in the diagnosis of tubal patency. Methods Forty patients with 4D-Hy Co Sy in Shenyang Maternal and Child Hospital from November 2014 to December 2015 were selected. Laparoscopic examination was used as the gold standard to evaluate tubal patency and to analyze 4D-Hy Co S diagnostic ability. Results There were 80 tubal tubes in 40 patients in this study. There was no significant difference between 4D-Hy Co Sy and laparoscopy (Kappa = 0.0769, P = 0.803). The sensitivity of 4D-Hy Co Sy for tubal obstruction was 92.0 % (46/50), the specificity was 90.0% (27/30), the correct rate was 91.2% (73/80). Conclusions 4D-Hy Co Sy can observe the shape and adhesion of the uterus and fallopian tubes stereoscopically, and the sensitivity, specificity and accuracy of the diagnosis of tubal patency are high, to a certain extent, instead of laparoscopy.
