目的:探讨非小细胞肺癌中细胞周期蛋白D1(cyclinD1)的表达与肿瘤生物学行为的关系。方法:采用免疫组织化学EnVision二步法对46例非小细胞肺癌手术标本进行cyclinD1检测,并结合临床随访资料作结果分析。结果:cyclinD1阳性表达率为54.3%(25/46),但与年龄、性别、TNM分期、组织类型及组织学分级均无相关性(P>0.05);临床随访资料显示,在cyclinD1基因表达阳性的25例患者中有10例发生远处转移,而cyclinD1表达阴性的21例中,仅1例发生远处转移,二者差别显著(P<0.05)。进一步分析还发现,随着cyclinD1阳性表达程度的增高,发生转移的机率也增大,表达中度阳性(++)的患者其转移发生率明显高于阴性(-)者(P<0.05),而表达强阳性(+++)者与阴性(-)者比较,其转移发生率差异非常显著(P<0.01)。结论:非小细胞肺癌cyclinD1过度表达与肿瘤是否发生远处转移有关;检测cyclinD1的表达有助于判断预后并指导治疗。
Objective: To investigate the relationship between the expression of cyclin D1 (cyclinD1) and tumor biological behavior in non-small cell lung cancer. Methods: EnVision two-step immunohistochemistry was used to detect cyclinD1 in 46 specimens of non-small cell lung cancer (NSCLC). The clinical follow-up data were used to analyze the results. Results: The positive expression rate of cyclinD1 was 54.3% (25/46), but there was no correlation with age, gender, TNM staging, histological type and histological grade (P>0.05). Clinical follow-up data showed positive expression in cyclinD1 gene. Among the 25 cases, distant metastasis occurred in 10 cases, but of the 21 cases with negative cyclinDl expression, only 1 case had distant metastasis, and the difference was significant (P<0.05). Further analysis also found that with the increased expression of cyclinD1, the chance of metastasis increased, and the incidence of metastasis was significantly higher in patients with moderately positive (++) than in those with negative (-) (P<0.05). There was a significant difference in the incidence of metastasis between those with strong positive (+++) and negative (-) (P<0.01). Conclusion: The overexpression of cyclinD1 in non-small cell lung cancer is related to the distant metastasis of tumors. Detecting the expression of cyclinD1 is helpful to predict the prognosis and guide the treatment.
