目的 :探讨bcl 2、p5 3、CathepsinD(CD)和前列腺特异性抗原 (PSA)表达与乳腺癌预后的关系。 方法 :应用免疫组化LSAB方法检测 6 4例乳腺癌及 30例乳腺良性病变的表达。分析bcl 2、p5 3、CD和PSA与乳腺癌组织学分级、腋淋巴结转移、复发和预后的关系。结果 :bcl 2、p5 3表达之间有显著性差异 (P <0 0 5 ) ,呈负相关 ;bcl 2与CD和PSA表达之间均无关 (P >0 0 5 )。bcl 2表达随组织学分级增加阳性率降低 ,p5 3则相反 ;CD和PSA表达与组织学分级无关 (P >0 0 5 )。p5 3和CD表达均与腋淋巴结转移呈正相关 (P <0 0 5 ) ,PSA表达则呈负相关 (P <0 0 5 ) ;bcl 2表达与腋淋巴结转移无关 (P >0 0 5 )。p5 3表达复发组明显高于无复发组 (P <0 0 5 ) ,bcl 2、CD和PSA表达与有无复发均无关 (P >0 0 5 )。p5 3和CD表达阳性率≤ 5年生存组均明显高于 >5年生存组 (P<0 0 5和 0 0 1) ,呈负相关 ;PSA表达则相反。bcl 2表达与生存期无关 (P >0 0 5 )。结论 :bcl 2表达与预后无关 ,其阳性表达可反映肿瘤属分化较好或属早期阶段。p5 3、CD和PSA均可单独作为乳腺癌的预后指标 ;p5 3和CD表达均与预后呈负相关 ,PSA表达与预后呈正相关。
Objective: To investigate the relationship between bcl-2, p53, CathepsinD (CD) and prostate specific antigen (PSA) expression and prognosis of breast cancer. Methods : Immunohistochemical LSAB method was used to detect the expression of 64 cases of breast cancer and 30 cases of benign breast lesions. The relationship between bcl-2, p53, CD and PSA and histological grade, axillary lymph node metastasis, recurrence, and prognosis of breast cancer was analyzed. Results: The expression of bcl 2 and p53 was significantly different (P <0 05), showing a negative correlation. Bcl 2 was not related to the expression of CD and PSA (P > 0 05). The positive rate of bcl-2 expression decreased with histological grade, while p53 was the opposite. The expression of CD and PSA was not associated with histological grade (P > 0.05). Both p53 and CD expression were positively correlated with axillary lymph node metastasis (P <0 05), and PSA expression was negatively correlated (P <0 05); bcl-2 expression was not associated with axillary lymph node metastasis (P > 0.05). The expression of p53 in the recurrence group was significantly higher than that in the non-recurrence group (P < 0.05). The expression of bcl-2, CD and PSA was not associated with recurrence (P > 0.05). The positive rate of p53 and CD expression in ≤ 5 years survival group was significantly higher than that in the >5 year survival group (P <0 05 and 0 01), showing a negative correlation; PSA expression was the opposite. Bcl 2 expression was not associated with survival (P > 0 05). Conclusion : The expression of bcl 2 has nothing to do with the prognosis. The positive expression of bcl 2 may reflect that the tumor is well differentiated or is in an early stage. P53, CD and PSA could be used as prognostic indicators of breast cancer. The expression of p53 and CD were negatively correlated with the prognosis. The expression of PSA was positively correlated with the prognosis.
