在大脑中动脉(MCA)缺血性卒中的急性期预测神经病学的结局是困难的。作者应用正电子体层摄影(PET)对急性MCA区域卒中的病人作了前瞻性研究,旨在观察脑灌注和氧消耗的改变形式能否预示神经病学进程。选择首次MCA卒中,病程<18h的病例,在当天和60天后采用Mathew法和Martinez-Vila指数(EI)对神经病学的缺陷进行评价,结局分为差、中、好。应用~(15)O平衡法获取灌注和氧消耗参数的PET影像,异常表现分为重度和中度灌注减少及灌注比对侧相对增多(自发性再灌注):重度和中度氧消耗降低。
Predicting the neurological outcome in the acute phase of ischemic stroke in the middle cerebral artery (MCA) is difficult. The authors prospectively studied the use of positron emission tomography (PET) in patients with acute stroke in the MCA region to see if changes in cerebral perfusion and oxygen consumption could predict neurological progression. The first MCA stroke was selected with a duration of <18 h. Neurological deficits were assessed using the Mathew method and the Martinez-Vila index (EI) on the day and after 60 days. The outcomes were classified as poor, moderate, and good. The PET images of perfusion and oxygen consumption parameters were obtained by the equilibrium method of ~ (15) O. The abnormal manifestations were divided into severe and moderate perfusion decrease and relative perfusion perfusion contrast (spontaneous reperfusion): the reduction of severe and moderate oxygen consumption.
