目的:分析下颌第一前磨牙C形牙根的根管横断面形态,并对最小根管壁厚度进行测量。方法:选取44颗具有C形牙根的下颌第一前磨牙,使用锥体束CT(cone-beam computed tomography,CBCT)进行扫描,并三维重建。分析根颈1/3、根中1/3和根尖1/3段的根管横断面形态分布变化,并对牙根各段的根面沟侧及对侧最小根管壁厚度进行测量比较。采用SPSS13.0软件包对数据进行统计学分析。结果:根中1/3、根尖1/3的根管横断面形态分布较为复杂,以C2~C4型为主,与根颈1/3存在显著差异(P<0.05);根中1/3和根尖1/3根面沟侧的最小根管壁厚度小于对侧(P<0.05),且最小根管壁厚度均<1 mm。结论:下颌第一前磨牙C形牙根的中下段根管形态复杂,且根面沟侧的根管壁较薄,临床治疗中应考虑到以上解剖特点。
OBJECTIVE: To analyze the morphology of root canal of C-shaped root of mandibular first premolar, and measure the thickness of the smallest root canal. Methods: Forty-four mandibular first premolar teeth with C-shaped roots were selected and scanned by cone-beam computed tomography (CBCT). Three-dimensional reconstruction was performed. The changes of root canal cross-sectional morphology distribution in root 1/3, root 1/3 and root apex 1/3 were analyzed. The root canal ditch and contralateral minimum root canal wall thickness were measured and compared. SPSS13.0 software package for statistical analysis of the data. Results: The morphology distribution of root canal in 1/3 and 1/3 root of root canal was more complicated with C2 ~ C4 type and 1/3 of root canal (P <0.05) 3 and apical 1/3 root ditch side of the smallest root canal wall thickness is less than the contralateral (P <0.05), and the minimum root canal wall thickness <1 mm. CONCLUSION: The shape of root canal in the middle and lower segments of the C-shaped root of mandibular first premolar is complex, and the root canal wall on the side of gutter is thin. The above anatomical features should be taken into account in clinical treatment.
