目的探讨正常人群碎裂QRS(fQRS)的分布特点。方法连续随机选取不同地域8672例社区人群的体检结果作为研究样本,分析体表心电图中fQRS的分布部位、性别及年龄流行病学分布特征。部位分布依据体表心电图确定:下壁导联(Ⅱ、Ⅲ、aVF)、前壁导联(V1-V5)、侧壁导联(Ⅰ、aVL、V6);研究人群每20岁作为1个年龄梯度,共分5个年龄段。结果人群年龄16~94岁,平均年龄(52.5±12.8)岁。心电图中fQRS波总发生率为21.49%,下壁、胸前和侧壁导联fQRS的发生率分别为13.13%、6.56%和6.61%,fQRS多发生于下壁导联,差异有统计学意义(P<0.001)。男性发生率为27.37%,女性为18.26%,差异有统计学意义(P<0.001);5个年龄段之间fQRS的发生率存在差异,随年龄增长fQRS发生率逐渐增加,且二者呈正相关(r=0.7864,P=0.005)。结论 fQRS的分布存在性别及年龄的差异,老年男性人群fQRS多发,且多出现于下壁导联。
Objective To investigate the distribution of QRS (fQRS) in normal population. Methods The physical examination results of 8672 community residents in different regions were randomly selected as the research samples to analyze the epidemiological distribution of fQRS distribution, sex and age in the body surface electrocardiogram. The site distribution was determined according to the body surface electrocardiogram: the inferior leads (Ⅱ, Ⅲ, aVF), the anterior leads (V1-V5), the lateral leads (Ⅰ, aVL, V6) Age gradient, is divided into 5 age groups. Results The population aged 16-94 years, mean age (52.5 ± 12.8) years. The total incidence of fQRS wave in ECG was 21.49%, and the incidence of fQRS in inferior wall, anterior chest wall and lateral wall was 13.13%, 6.56% and 6.61%, respectively. The fQRS occurred mostly in inferior wall leads with statistical significance (P <0.001). The incidence of males and females was 27.37% and 18.26%, respectively (P <0.001). There was a significant difference in the incidence of fQRS between the five age groups. The incidence of fQRS increased with age, and there was a positive correlation between them (r = 0.7864, P = 0.005). Conclusions The distribution of fQRS has gender and age differences. The fQRS is frequent in the elderly male population and appears mostly in the inferior leads.
