室性早搏延期代偿是指室性早搏后较远的窦性P波不能下传心室,而与室早基本同时(可略有先后)发生的窦性P波却能缓慢地下传心室(颇似间位性室性早搏),现报告一例。 患者 男,18岁,临床诊断:病毒性心肌炎。心电图(附图)为Ⅱ导联续描记于1992年10月6日,P波直立,略有不齐,大部分后继qR型波。ⅡbP_(10)提前发生,负正双向,其后亦继以qR型波,PR间期0.14s。另可见频发室性早搏,呈宽大畸形R型波,T波与主波方向相反,除Ⅱ_aR_(13)、Ⅱ_bR_(11)各干扰一次窦性P波下传心室形成完全性代偿间期,Ⅱ_aR_(10)因其发生略迟,而其前窦P波又发生略早致使二者形成室性融合波外,Ⅱ_aR_(15、7)亦为室早,但
