目的:探讨亚油酸(Linoleic acid,LA)对维持性血液透析(MHD)患者体内非对称性二甲基精氨酸(ADMA)水平和白介素-6(IL-6)水平的影响,进一步明确LA在血透患者并发心血管疾病中的作用。方法:根据入选标准随机选取血液透析患者106名。均给予标准化蛋白质1.2~1.5 g/(kg·d),碳水化合物5~6 g/(kg·d),脂肪1.3~1.7 g/(kg·d)饮食,在此基础上53人采用葵花籽油烹饪食物(实验组),其余53名血透患者采用普通食用油烹饪食物(对照组)。酶联免疫法(ELISA)检测实验前(入选之日)和实验后(3月末)各组患者循环中ADMA和IL-6的水平。气液色谱法分别检测实验前后血浆LA的浓度。结果:实验组中,实验后ADMA和IL-6的水平明显降低,且明显低于对照组(P<0.05)。相关性分析结果显示LA与ADMA(r=-0.434,P<0.01)、与IL-6(r=-0.467,P<0.01)成负相关。心血管事件(CVE)的发生率统计结果为实验组CVE发生率为32.18%,对照组CVE发生率为67.82%。结论:给予葵花籽油的干预过程,血浆中LA的水平显著升高,LA可以明显降低血透患者循环中ADMA和IL-6的水平,减少CVE的发生,有望成为血透患者发生心血管疾病的预防措施。
