目的:探讨上皮细胞黏附分子(Ep CAM)和细胞角蛋白19(CK19)在胃癌患者中的阳性率及其与胃癌转移和预后的相关性。方法:选择2012年1月至2013年2月在我院行胃镜检查确诊胃癌并在胃肠外科行根治性胃癌切除手术的105例胃癌患者作为观察组,同期选取40例浅表性胃炎患者作为对照组,将所获得的胃组织标本制成组织芯片,通过免疫组化技术对两组组织芯片Ep CAM和CK19的表达进行检测,并对比两组阳性率的差异,同时分析观察组中Ep CAM和CK19的表达与胃癌的转移及预后的相关性。结果:观察组Ep CAM和CK19阳性率均明显高于对照组(χ2为35.282、15.137,均P<0.05)。Ep CAM与肿瘤浸润程度、淋巴结转移情况、远处转移以及组织分化程度呈正相关(r为0.585、0.514、0.623、1.025,均P<0.05);CK19与上述各因素无相关性(P>0.05)。Ep CAM与患者预后呈负相关(r=-0.605,P<0.05);CK19与患者预后无相关性(P>0.05)。结论:Ep CAM和CK19在癌变胃组织中的阳性率显著高于未癌变的胃组织,且Ep CAM与胃癌的转移呈正相关,与其预后呈负相关。
Objective: To investigate the positive rate of epithelial cell adhesion molecule (Ep CAM) and cytokeratin 19 (CK19) in gastric cancer and its correlation with the metastasis and prognosis of gastric cancer. Methods: From January 2012 to February 2013 in our hospital gastroscopy diagnosis of gastric cancer and gastric cancer radical resection of gastric cancer in 105 cases of gastric cancer patients as the observation group, the same period selected 40 cases of superficial gastritis patients as In the control group, the obtained gastric tissue samples were made into tissue microarray, and the expression of Ep CAM and CK19 in two groups of tissues was detected by immunohistochemistry. The difference of the positive rate of the two groups was also compared. At the same time, Ep CAM And CK19 expression and gastric cancer metastasis and prognosis. Results: The positive rates of Ep CAM and CK19 in the observation group were significantly higher than those in the control group (χ2 = 35.282 and 15.137, all P <0.05). There was a positive correlation between Ep CAM and tumor infiltration, lymph node metastasis, distant metastasis and histological differentiation (r = 0.585,0.514,0.623,1.025, all P <0.05). There was no correlation between Ep CAM and other factors (P> 0.05) . Ep CAM was negatively correlated with patient prognosis (r = -0.605, P <0.05). There was no correlation between CK19 and prognosis (P> 0.05). CONCLUSION: The positive rates of Ep CAM and CK19 in cancerous gastric tissue are significantly higher than those in non-cancerous gastric tissue. Ep CAM is positively correlated with gastric cancer metastasis and negatively correlated with prognosis.
