Objective: The Eagles syndrome is characterized by a variety of symptoms including throat pain, pharyngeal foreign body sensation, dysphagia, referred otalgia, and neck and throat pain exacerbated by head rotations.Styloid process longer than 25mm is considered elongated and usually responsible for Eagles syndrome.In our case, we reported a patient with a rare giant styloid process which was approximately 81.7mm.He complained a rare symptom of hemi-tongue paresthesia.Methods: Surgical resection of the elongated styloid is a routine treatment and can be accomplished through either a transoral or extraoral approach.In our case, we selected the extraoral approach.Results: The elongated styloid process was removed by extraoral approach.And the symptom of hemi-tongue paresthesia was alleviated.Conclusions: We concluded that if the length of styloid process was medium to large, the extraoral approach was advocated.
