目的探讨东营市河口区上消化道癌症的临床流行病学特点。方法选择2000年1月至2008年1月于东营市河口区二所医院进行胃镜检查并经病理确诊的上消化道癌症患者,对其临床、病理、内镜及流行病学特征进行分析。结果①8年内上消化道癌症总检出率为9.10%,其中食管癌检出率4.12%,非贲门部胃癌检出率4.00%,贲门癌检出率0.95%%,十二指肠癌检出率0.03%。食管癌中,13.4%为食管腺癌、86.6%为鳞状细胞癌。检出的上消化道癌症以进展期为主,1.96%(24/1226)为早期癌症。②男性发病率(76.8%)显著高于女性(23.2%)(P<0.01);男性(58.1±14.2)与女性(54.7±15.3)年龄相比无显著性差别(P>0.05);上消化道癌症中,≥50岁者1873例(69.1%)显著高于<50岁者(836例,30.9%)(P<0.01)。结论8年来东营市河口区上消化道癌症由高到低排序为食管癌、非贲门部胃癌、贲门癌及十二指肠癌,食管癌与非贲门胃癌检出率无显著性差异,早期癌症的检出率较低,应当引起足够的重视。
Objective To investigate the clinical epidemiological characteristics of upper gastrointestinal cancer in the estuary of Dongying City. Methods The clinical, pathological, endoscopic and epidemiological characteristics of upper gastrointestinal cancer patients who underwent gastroscopy and pathologically diagnosed at two hospitals in Hekou District, Dongying City from January 2000 to January 2008 were analyzed. Results ① The total detection rate of upper gastrointestinal cancer in 9 years was 9.10%, of which the detection rate of esophageal cancer was 4.12%, the detection rate of gastric cancer in non-cardia department was 4.00%, the detection rate of cardia cancer was 0.95 %%, and the detection rate of duodenal cancer 0.03%. Esophageal cancer, 13.4% of esophageal adenocarcinoma, 86.6% of squamous cell carcinoma. Upper gastrointestinal cancers were detected as predominantly advanced, and 1.96% (24/1226) were early cancers. ②The incidence of male was significantly higher than that of female (76.8%) (23.2%) (P <0.01); there was no significant difference between male (58.1 ± 14.2) and female (54.7 ± 15.3) In cancer of the cancers, 1873 cases (≥ 69.1%) were more than 50 years old (836 cases, 30.9%) (P <0.01). Conclusion The esophageal cancer, non-cardia gastric cancer, cardia cancer and duodenal cancer, esophageal cancer and non-cardia gastric cancer have no significant difference in cancers of upper gastrointestinal tract in the estuary of Dongying City in the past eight years. The incidence of early cancer The detection rate is low, should cause enough attention.
