肺内黑色素瘤是一种少见疾病,本文一例为两肺内弥漫性黑色素瘤,更为罕见。 患者女性,32岁,于1991年7月11日因两肺大小不等结节状阴影被收住我院内科诊治。患者从6岁开始。在鼻周围、前额、双侧颊部陆续出现绿豆大小、坚韧、淡黄色的丘疹,不痛不痒,无症状。10岁开始丘疹增多,该处皮肤转红,见淡褐色毛细血管。患者曾生育1女,现已8岁。女儿从6岁开始。面颊部也有类似丘疹出现,亦无症状,智力正常,无癫癎发作。患者在入院前1个月曾出现刺激性咳嗽,放射线检查发现两肺呈弥漫性大小不等的结节状阴影,伴肺门增大。入院后检查:患者全身皮肤、会阴和眼结膜均未见痣与黑色块物。住院期间,发现两肺阴影有缓慢增大和融合趋势,在右颈部出现两个蚕豆大小的淋巴结,中等硬度,固定。纤维支气管镜检查发现,右肺下叶基底段管腔内有灰黑色块物阻塞,取块物经活组织检查见黑色素细胞。1991年8月8 Pulmonary melanoma is a rare disease. In this case, diffuse lung melanoma is more common in both lungs. The female patient, 32 years old, was admitted to our hospital for medical treatment on July 11, 1991 due to the size of the nodules in both lungs. The patient started at the age of 6. Green bean size, tough, pale yellow papules appeared gradually around the nose, forehead, and both sides of the cheeks. It was superficial and asymptomatic. At the age of 10, the number of pimples increased, the skin turned red, and light brown capillaries were seen. The patient had 1 child and is now 8 years old. The daughter starts from 6 years old. There are similar pimples on the cheeks, as well as asymptomatic, normal intelligence, and no epileptic seizures. The patient had irritating cough 1 month before admission. Radiographic examination revealed diffusely diffuse nodular shades of the two lungs with enlarged hilars. Post-admission examination: The patient’s general skin, perineum, and conjunctiva of the patient did not see blemishes or black masses. During hospitalization, it was found that the shadows of the two lungs slowly increased and converged, and two broad bean-sized lymph nodes appeared in the right neck, with medium hardness and fixation. Fiberoptic bronchoscopy revealed obstruction of gray-black masses in the lumen of the basement of the lower lobe of the right lung. The melanoma cells were removed from the mass by biopsy.
