背景:激光可用于高效外科手术止血,其在外伤止血方面也具有广泛的应用前景。目的:通过测定止血时间和3种反映凝血机制指标的变化情况以及进行组织化学染色,对532nm激光局部照射的止血作用进行考察。材料和方法:10只成年雌性健康的新西兰大耳白兔,体重(2.5~3.0)kg。10只家兔左右耳进行对照实验。在耳背距耳尖5cm处中央作(0.6~0.8)cm长,深及中央动静脉的横切口(左右两耳被切血管粗细相近)。左耳传统组,用传统的压迫伤口止血法止血;右耳为激光照射组,用5W 532nm激光照射伤口后再按压止血。用秒表测量止血时间(s)。止血后取心脏处血液测试凝血酶原时间(PT),活化部分凝血活酶时间(APTT)和血小板(PLT)数量。伤后第3天、第7天、第10天、第20天和第30天对伤口取活检,做常规苏木精-伊红染色,在普通光学显微镜下观察组织形态变化。结果:10只家兔均进入结果分析。①与传统组对照,激光照射组伤口止血速度快,差异具有显著性意义(P<0.05)。②与传统组对照,激光照射组PT时间增长,APTT时间缩短,PLT数量增加,三种指标差异有统计学意义(P<0.05)。③与传统组对照激光照射组组织损伤加重,伤口愈合时间更长。
Background: The laser can be used for high-efficiency surgical hemostasis, which also has a wide range of applications in the traumatic hemostasis. OBJECTIVE: To investigate the hemostatic effects of 532nm laser local irradiation by measuring the time of hemostasis and three kinds of changes reflecting coagulation mechanism and histochemical staining. MATERIAL AND METHODS: Ten adult female healthy New Zealand white rabbits weighing 2.5-3.0 kg. 10 rabbits left and right ears for control experiments. In the ear away from the tip 5cm at the center for (0.6 ~ 0.8) cm length, depth and central artery and vein transverse incision (left and right ears are cut into similar thickness). The left ear traditional group, with the traditional oppression wound to stop bleeding; right ear laser irradiation group, with 5W 532nm laser irradiation and then stop the bleeding. Stop the bleeding time with a stop watch (s). The hematocrit at the heart was taken after hemostasis and the prothrombin time (PT), activated partial thromboplastin time (APTT) and platelet count (PLT) were measured. The wounds were biopsied on day 3, day 7, day 10, day 20 and day 30, and routine hematoxylin-eosin staining was performed. Morphological changes were observed under a normal light microscope. Results: All 10 rabbits were involved in the result analysis. ① Compared with the traditional group, the wound speed of laser irradiation group was fast, the difference was significant (P <0.05). ② Compared with the traditional group, the PT of laser irradiation group increased, the APTT time shortened, and the number of PLT increased. The difference of the three indexes was statistically significant (P <0.05). ③Compared with the traditional group, the tissue damage of the laser irradiation group increased, the wound healing time was longer.
