目的 探讨肥厚型心肌病、高血压病患者与正常人左室后壁、室间隔心肌声学特性是否存在差异 ,以期将超声组织定征作为鉴别诊断的指标之一。方法 将被检者分成 3组 :肥厚型心肌病组 (HCM组 )、高血压性心脏病组 (EH组 )及正常对照组 ,分别测定各组左室后壁、室间隔部位的心肌背向散射参数。结果 左室后壁心肌 :HCM组与EH组的IBS、CVIB无显著性差异 (P >0 0 5 ) ,而与正常对照组均有差异 (P <0 0 5 ) ;室间隔部位 3组IBS、CVIB均有显著性差异 (P <0 0 5 ) ;HCM组室间隔、左室后壁均存在IBS、CVIB的跨壁梯度 ,而EH组、正常对照组则无上述异常存在。结论 HCM患者左室后壁心肌的声学特性与正常对照组有差异 ,室间隔与左室后壁均存在IBS、CVIB的跨壁梯度。因此背向散射参数测定可作为鉴别肥厚型心肌病和高血压性心脏病的一项指标
OBJECTIVE: To investigate whether there is any difference in the acoustic properties between hypertrophic cardiomyopathy, hypertensive patients and normal persons, and whether there is any difference in the acoustic properties between the left ventricular posterior wall and the interventricular septal myocardium. Methods The subjects were divided into three groups: hypertrophic cardiomyopathy group (HCM group), hypertensive heart disease group (EH group) and normal control group. The left ventricular posterior wall and ventricular septal myocardial dorsal Scattering parameters. Results There were no significant differences in IBS and CVIB between HCM group and EH group (P> 0.05), but there were significant differences between them in normal group and control group (P <0.05) (P <0.05). There was a transmural gradient of IBS and CVIB in the interventricular septum and the posterior wall of the left ventricle in HCM group, while there was no abnormality in the EH group and the normal control group. Conclusion The acoustic properties of left ventricular posterior wall myocardium in HCM patients are different from those in normal control group. The transmural gradients of IBS and CVIB exist in both ventricular septum and left posterior wall. Therefore, backscatter parameters can be used as a marker to identify hypertrophic cardiomyopathy and hypertensive heart disease
