目的探讨利用用药90%(DU90%)国际指标分析骨科围术期预防用抗菌药物临床应用情况。方法选取2011年1—5月蚌埠市第三人民医院出院的骨科手术患者597例,另选取2014年1—5月蚌埠市第三人民医院出院的骨科手术患者641例;排除合并有其他感染性疾病者。采用世界卫生组织(WHO)推荐的限定日剂量(DDD)计算抗菌药物用药频度(DDDs),抗菌药物DDD参考《中国药典临床用药须知》2010年版及卫生部抗菌药物监测网药品字典。结果 2014年1—5月蚌埠市第三人民医院抗菌药物使用率、抗菌药物使用强度(AUD)、抗菌药物费用占药费总额的百分比、住院患者人均使用抗菌药物费用低于2011年1—5月;2011年1—5月蚌埠市第三人民医院骨科预防用抗菌药物品种为60种,其中DU90%的有17种,DDDs(DU90%部分)占全部抗菌药物品种的28.33%;2014年1—5月蚌埠市第三人民医院骨科预防用抗菌药物品种为19种,其中DU90%的有8种,DDDs(DU90%部分)占全部抗菌药物品种的42.11%;2014年1—5月DU90%部分第一、二代头孢菌素及非限制性抗菌药物的DDDs高于2011年1—5月;2014年1—5月第一、二代头孢菌素及非限制抗菌药物的日用药金额(DDC)小于2011年1—5月。结论 DU90%可作为医疗机构药物利用评估的有效工具,本院抗菌药物使用基本合理。
Objective To explore the clinical application of antimicrobial agents for preventive perioperative orthopedics by using 90% (DU90%) international indexes. Methods A total of 597 orthopedic patients discharged from the Third People’s Hospital of Bengbu from January to May 2011 were selected. Another 641 patients undergoing orthopedic surgery were discharged from the Third People’s Hospital of Bengbu from January to May in 2014. Other infectious diseases were excluded Disease. According to the World Health Organization (WHO) recommended daily dose (DDD) to calculate the frequency of antimicrobial drugs (DDDs), antimicrobial DDD reference “Chinese Pharmacopoeia Clinical Notes 2010” and the Ministry of Health antimicrobial drug monitoring network drug dictionary. Results From January to May 2014, the usage rate of antimicrobial agents, the strength of antimicrobial agents used in the Third People’s Hospital of Bengbu City, the percentage of antimicrobial drugs in the total amount of anti-bacterial drugs, the average cost of antimicrobial drugs in inpatients were lower than those of 1-5 Month; 2011 January-May Bengbu City Third People’s Hospital orthopedic prophylaxis with antimicrobial species 60 species, of which DU90% of 17 species, DDDs (DU90% part) accounted for 28.33% of all antimicrobial drugs; 2014 1 - May Bengbu City Third People’s Hospital orthopedic prophylaxis with 19 kinds of antibacterial drugs, of which 8 kinds of DU90%, DDDs (DU90% part) accounted for 42.11% of all antimicrobial drugs; 2014 January-May DU90% Some of the first and second generation cephalosporins and non-limiting antibacterial drugs DDDs higher than January-May 2011; January-May 2014 first and second-generation cephalosporins and non-limiting antibacterial drugs daily dose DDC) is less than January-May 2011. Conclusions DU90% can be used as an effective tool for the evaluation of drug utilization in medical institutions. The use of antimicrobial drugs in our hospital is basically reasonable.
