目的探讨检测外周静脉血液中游离肿瘤细胞表达的 CD44v mRNA 对原发性肝癌的辅助诊断以及预测、监测术后复发的意义。方法应用套式 PCR 方法检测原发性肝癌患者外周静脉血液中 CD44v mRNA 的表达,并结合病理检查及随访结果探讨其意义。结果 15例原发性肝癌患者外周静脉血液中检出了 CD44v mRNA 10例(66.67%,10/15)。随访回报13例,CD44v mRNA 阳性9例,阴性4例,前者侵袭转移相关的病理指标高于后者(P<0.005),复发率亦与此一致。结论检测外周静脉血液中 CD44v 的表达能够作为原发性肝癌患者的鉴别诊断及预测、监测其复发的辅助手段。
Objective To explore the significance of detecting CD44v mRNA expressed by free tumor cells in peripheral venous blood to assist in the diagnosis of primary hepatocellular carcinoma, and to predict and monitor postoperative recurrence. Methods The expression of CD44v mRNA in peripheral blood of patients with primary hepatocellular carcinoma was detected by nested PCR method. The significance of CD44v mRNA in combination with pathological examination and follow-up results was explored. Results Ten cases (66.67%, 10/15) of CD44v mRNA were detected in the peripheral venous blood of 15 patients with primary liver cancer. Follow-up results were obtained in 13 patients. There were 9 cases of CD44v mRNA positive and 4 cases of negative. The pathological parameters associated with invasion and metastasis of the former were higher than those of the latter (P<0.005). The recurrence rate was also consistent with this. Conclusion The detection of CD44v expression in peripheral venous blood can be used as an aid in the differential diagnosis, prediction and monitoring of recurrence of primary liver cancer patients.
