目的:研究实时三维超声在胎儿畸形诊断中的应用价值。方法:以2008年1月~2012年3月在上虞市人民医院进行孕期保健检查以及在其他医院初诊为可疑胎儿结构的2 875例孕妇为研究对象,采用实时三维超声实施二维及三维超声检查,对检查结果和引产结果进行比较分析。结果:2 875例孕12~40周孕妇中,经引产及出生证实为畸形的胎儿有133例,产前二维超声检测出128例,漏诊5例,诊断符合率为96.97%。采用二维超声进行胎儿畸形诊断的敏感度为96.97%,特异度为100.00%,准确度为99.82%,阴性预测值为99.82%,阳性预测值为100.00%。采用实时三维超声在产前检测出132例,仅漏诊1例,诊断符合率为99.25%,诊断敏感度达到99.25%,特异度100.00%,准确度达到99.96%,阴性预测值为99.96%,阳性预测值为100.00%。结论:采用实时三维超声用于胎儿畸形诊断敏感度高、特异性好,具有较好的应用价值。
Objective: To study the value of real-time three-dimensional ultrasound in the diagnosis of fetal malformations. Methods: From January 2008 to March 2012 in Shangyu People’s Hospital during pregnancy health check-up and other fetuses in other hospitals for the first 287 cases of suspected fetal structure of pregnant women as the research object, the use of real-time three-dimensional ultrasound implementation of two-dimensional and three-dimensional ultrasound , The test results and the results of abortion comparative analysis. Using real-time three-dimensional ultrasound in prenatal detection of 132 cases, only one missed diagnosis, the diagnostic coincidence rate was 99.25%, the diagnostic sensitivity of 99.25%, specificity of 100.00%, the accuracy of 99.96%, the negative predictive value of 99.96%, positive The forecast is 100.00%. Conclusion: The real-time three-dimensional ultrasound is used in the diagnosis of fetal malformations with high sensitivity and good specificity, and has good application value.
