目的:探讨胃癌中黏蛋白1(MUC1)异常高表达与胃癌临床分期、组织学类型、肿瘤发生部位、肿瘤大小和细胞增殖活性等临床病理学关系。方法:收集手术切除胃癌标本61例,应用HE染色切片进行组织病理学分类与核分裂指数评价,EnVision两步法行MUC1与Ki-67单克隆抗体免疫组织化学染色,10%以上肿瘤细胞出现MUC1胞质着色视为高表达;40%以上肿瘤细胞核出现Ki-67阳性视为Ki-67标记指数增高。结果:32例胃癌(52.5%)有MUC1高表达,MUC1高表达组核分裂指数明显高于MUC1阴性组(P=0.027);胃底贲门部胃癌MUC1高表达病例较胃体幽门部胃癌相对多见(P=0.068);MUCI高表达与否与Ki-67标记指数高低一致率达81.9%。Ki-67标记指数与核分裂指数一致率可达75.4%。结论:MUC1高表达与胃癌细胞增殖活性增高有关,可将其作为反映胃癌增殖活性的有用标记物。
Objective: To investigate the clinicopathological relationship between the abnormal expression of MUC1 in gastric cancer and clinical staging, histological type, tumor location, tumor size and cell proliferation activity in gastric cancer. Methods: Totally 61 gastric cancer specimens were collected for histological classification and mitotic index evaluation by HE staining. MUC1 and Ki-67 monoclonal antibodies were immunohistochemically stained with EnVision in two steps. MUC1 cells were found in more than 10% of the tumor cells Qualitative staining as high expression; more than 40% of the tumor nuclei showed Ki-67 positive Ki-67 marker index increased. Results: MUC1 was highly expressed in 32 cases of gastric cancer (52.5%). The mitotic index of MUC1 overexpression group was significantly higher than that of MUC1 negative group (P = 0.027). MUC1 overexpression in gastric cardia gastric carcinoma was more common than gastric pyloric gastric carcinoma (P = 0.068). The concordance rate between MUCI high expression and Ki-67 labeling index was 81.9%. Ki-67 labeling index and mitotic index consistency rate of up to 75.4%. Conclusion: MUC1 overexpression is associated with increased proliferation activity of gastric cancer cells, which can be used as a useful marker of gastric cancer proliferation.
