庆大霉素是氨基甙类中抗菌范围广、应用最普遍的一种抗生素,遗憾的是它具耳、肾毒性,其发生率与血药浓度有密切关系。一般认为,庆大霉素的耳毒性与峰浓度(给药后能达到的最高血药浓度)的关系尚不明确,与谷浓度(指再次用药前测得的血药浓度)的关系密切。它对肾脏的毒性与血药浓度的相关性是复杂的,因为用药前患者原有的肾功能状态,以及用药后对肾脏产生损害的程度,都会直接影响血药水平。概括地说:肾功能不全时,血药浓度相对升高;血药浓度升高,肾脏愈加受害,形成恶性循环。
