目的 研究不同性质大肠粘膜细胞中细胞凋亡与增殖状态及其意义。方法 取正常大肠粘膜 10例 ,大肠腺瘤 2 0例 ,大肠癌远旁、近旁和癌粘膜各 3 0例。应用TUNEL、PCNA免疫组化染色 ,在连续切片上同步检测细胞凋亡和增殖状态。结果 TUNEL染色阳性物质定位于细胞核 ,固缩成团、聚成环状或形成凋亡小体。PCNA染色阳性物质也主要定位于细胞核 ,呈弥漫或颗粒状着色。正常大肠粘膜、大肠腺瘤和大肠癌细胞凋亡率 ,各组间比较均有统计学意义 (P <0 .0 1) ;PCNA阳性细胞数 ,各组间比较均有极显著性差异 (P <0 .0 1)。轻、中、重度不典型增生大肠腺瘤细胞凋亡率比较 ,轻度与中度、重度不典型增生比较有显著性差异 (P <0 .0 1) ;PCNA阳性细胞数 ,各组间比较均有极显著性差异(P <0 .0 1)。大肠癌远旁、近旁和癌组织中细胞凋亡率 ,各组间比较均有极显著性差异 (P <0 .0 1) ;PCNA阳性细胞数 ,各组间比较也均有极显著性差异 (P <0 .0 1)。细胞凋亡与增殖与大肠癌的分化程度、分期和淋巴结转移有关 ;与组织学类型无关。 3 0例大肠癌细胞凋亡率和PCNA的表达呈极显著性正相关关系 (P <0 .0 0 1)。 2 0例大肠腺瘤细胞凋亡率和PCNA的表达呈正相关关系 (P <0 .0 1)。结论 细胞凋亡与增殖是反映细胞周转率的指标 ;检测细胞凋?
Objective To study the apoptotic and proliferative states and their significance in different characteristics of colorectal mucosa cells. Methods 10 cases of normal colorectal mucosa, 20 cases of colorectal adenoma, 30 cases of distant colorectal cancer, proximal and adjacent mucosa, and 30 cases of cancerous mucosa. TUNEL and PCNA immunohistochemical staining were used to detect the apoptosis and proliferation of cells simultaneously on serial sections. RESULTS: Positive TUNEL staining was localized in the nucleus, condensed into clusters, aggregated into loops or formed apoptotic bodies. The PCNA staining positive material was also mainly localized in the nucleus and showed diffuse or granular staining. The apoptotic rates of normal colorectal mucosa, colorectal adenoma, and colorectal cancer cells were statistically significant among the groups (P < 0.01); the number of PCNA-positive cells was significantly different between the groups (P < 0.01). <0. 0 1). The apoptotic rate of mild, moderate and severe atypical hyperplasia colorectal adenoma cells was significantly different between mild and moderate and severe atypical hyperplasia (P < 0.01); the number of PCNA positive cells was compared between groups. All had significant differences (P < 0.01). The apoptosis rate in the distant, adjacent, and cancerous tissues of colorectal cancer was significantly different between the groups (P < 0.01); the number of PCNA-positive cells was also significantly different between the groups. (P < 0.01). Apoptosis and proliferation were related to the degree of differentiation, staging, and lymph node metastasis of colorectal cancer; they were not associated with histological type. A significant positive correlation was found between apoptosis rate and PCNA expression in 30 colorectal cancer cells (P < 0.01). There was a positive correlation between apoptosis rate and PCNA expression in 20 colorectal adenoma cells (P < 0.01). Conclusions Apoptosis and proliferation are indicators of cell turnover rate; cell death is detected.
