目的研究补充一剂碘的哺乳期妇女母乳-乳汁-婴儿的碘代谢过程。方法在阿克苏地区库车县牙哈镇4个村选择109名近半年内未服用过碘油丸的19~36岁哺乳期妇女及20日龄至18月龄婴幼儿,服药后的第1、3、7、14、30、50、75、100天分别采集实验组及对照组的哺乳妇女尿碘、乳汁及婴幼儿尿样,进行碘含量测定。结果哺乳期妇女尿碘及乳汁碘与婴幼儿尿碘之间呈正相关。经统计学检验,婴幼儿尿碘与哺乳期妇女尿碘呈正相关,相关系数(r)=0.773,P<0.001;对照组哺乳期妇女尿碘与婴幼儿尿碘之间r=0.526,P<0.001;婴幼儿尿碘与哺乳期妇女乳汁碘呈正相关,r=0.389,P<0.001;对照组哺乳期妇女乳汁碘与婴幼儿尿碘之间r=0.307,P<0.001。结论婴幼儿的碘来源主要是乳汁,哺乳妇女的碘营养状况直接影响婴幼儿。库车牙哈镇哺乳期妇女通过口服200 mg碘油胶丸可对婴幼儿尿碘最长保护期为100 d,乳母口服200 mg碘油胶丸对库车牙哈镇哺乳期妇女尿碘最长有效保护期不到50 d;可通过乳汁分泌到婴幼儿的最长保护期为100 d。
Objective To study the iodine metabolism of breast-milk-infant in breast-feeding women who received iodine supplementation. Methods A total of 109 breast-feeding women aged 19-36 years and infants from 20 days to 18 months who had not taken iodized oil in the past six months were enrolled in 4 villages of Yaha Town, Kuqa County, Aksu Prefecture. The first, The urinary iodine, milk and infant urine samples of lactating women in experimental group and control group were collected at 3, 7, 14, 30, 50, 75 and 100 days respectively for determination of iodine. Results In lactation women urinary iodine and milk iodine and urinary iodine was positively correlated. There was a positive correlation between urinary iodine and urinary iodine in infants and young children (r = 0.773, P <0.001). Urinary iodine of infants and young children in the control group was (r = 0.526, P < 0.001). The urinary iodine of infants and young children was positively correlated with the milk iodine of lactating women (r = 0.389, P <0.001). The lactation iodine of infants and young children in the control group was r = 0.307, P <0.001. Conclusion The main source of iodine in infants and young children is milk. The iodine nutrition status of lactating women directly affects infants and young children.
