目的探讨手术治疗配合手法整复治疗小儿肱骨髁上骨折的临床效果。方法选取2014年7月至2015年7月小儿肱骨髁上骨折患者89例,随机分为对照组40例和观察组49例,对照组患者实施肱三头肌外侧入路术治疗,观察组在对照组手术基础上配合手法整复治疗。比较两组手术基本情况,包括手术时间、手术出血量、术后24 h和72 h疼痛[采用用视觉模拟疼痛评分法(VAS)进行评估],并随访6个月了解两组治疗效果。结果与对照组相比,观察组手术时间为(35.59±10.49)min,显著较短(P<0.01),术中出血量明显为(49.40±9.60)ml,明显较少(P<0.05),术后24 h、72 h VAS评分为(4.14±1.56)分、(2.11±1.02)分,明显较低(P<0.05);观察组优良率为91.84%,显著高于对照组的75.00%(P<0.05)。结论手术治疗配合手法整复治疗小儿肱骨髁上骨折的临床效果显著优于单纯手术治疗,且可明显缩短手术时间、减少手术失血量、缓解手术疼痛,对小儿肱骨髁上骨折患者的恢复有重要意义。
Objective To investigate the clinical effects of surgical treatment and manipulation in the treatment of supracondylar humerus fractures in children. Methods From July 2014 to July 2015, 89 patients with supracondylar fractures of the humerus were randomly divided into control group (n = 40) and observation group (n = 49). Patients in the control group were treated with lateral triceps brachial approach. The observation group The control group based on the operation with plastic surgery. The basic conditions of the two groups were compared, including the operation time, the amount of bleeding during operation, the pain at 24 h and 72 h postoperatively [assessed by visual analogue pain scale (VAS)] and followed up for 6 months to understand the effect of the two groups. Results Compared with the control group, the operative time in the observation group was (35.59 ± 10.49) min, significantly shorter (P <0.01), and the bleeding volume in the observation group was significantly (49.40 ± 9.60) ml, significantly less (P <0.05) The VAS score at 24 h and 72 h after operation was (4.14 ± 1.56) and (2.11 ± 1.02) points, significantly lower (P <0.05). The excellent and good rate in the observation group was 91.84%, which was significantly higher than that in the control group (75.00% P <0.05). Conclusions Surgical treatment combined with manipulative method in the treatment of supracondylar humerus fractures in children is superior to simple surgical treatment, and can shorten the operation time, reduce the amount of blood loss and relieve the operation pain, and is important for the recovery of supracondylar humerus fractures in children significance.
