目的探讨不同组织学类型Kaposi肉瘤(KS)的表型特征。方法用形态学、免疫组化、原位杂交和DNA倍体分析方法对不同组织学类型30例共38份样本KS的表型特征作了观察。结果根据KS病变组织内血管成分和梭形细胞的比例分为血管瘤样型(早期)11份、梭形细胞型(晚期)14份和混合型(中期)13份。CD34和FⅧRAg在血管瘤样型表达强,梭形细胞型表达弱(P<0.01)。p53和c-met蛋白在血管瘤样型表达弱,梭形细胞型表达强(P<0.01);c-metmRNA表达与c-met蛋白相同,p53mRNA表达在血管瘤样型高于梭形细胞型。c-erbB-2在梭形细胞的表达高于血管瘤样型(P<0.01)。梭形细胞型PCNA指数高于血管瘤样型(P<0.01)。DNA倍体分析结果显示KS细胞皆为双倍体。结论血管瘤样型可能为良性病变,梭形细胞型为恶性病变,混合型为良性向恶性的过渡性病变或称交界性病变。
Objective To investigate the phenotypic characteristics of Kaposi’s sarcoma (KS) with different histological types. Methods Morphological, immunohistochemical, in situ hybridization and DNA ploidy analysis were used to observe the phenotypic characteristics of KS in 30 samples from 30 different histological types. The results were divided into 11 types of hemangiomas (early stage), 14 spindle-shaped (late stage) and 13 mixed (medium) stages according to the proportion of vascular components and spindle cells in KS lesions. CD34 and F Ⅷ RAg were strongly expressed in hemangiomas and were weakly expressed in spindle cells (P <0.01). The expression of p53 and c-met protein in hemangiomas was weak and spindle cell type was strong (P <0.01). The expression of c-met mRNA was the same as that of c-met protein. The expression of p53 mRNA in hemangiomas was higher than that of spindle cells . The expression of c-erbB-2 in spindle cells was higher than that in hemangiomas (P <0.01). The spindle cell PCNA index was higher than that of hemangiomas (P <0.01). DNA ploidy analysis showed that KS cells are all diploid. Conclusions Hemangiomas may be benign, spindle cell type may be malignant, and mixed may be benign or malignant transitional or borderline lesions.
