目的探讨肾上腺素在心肺复苏中的临床效果及作用机制。方法整取抽样2016年6月-2017年2月贵阳市第二人民医院行心肺复苏治疗的心脏骤停患者60例,静脉推注肾上腺素1mg后,改为静脉推注肾上腺素1mg+生理盐水10ml,经心肺复苏治疗后,立即送ICU病房治疗,监测所有患者使用肾上腺素前后主动脉收缩压(ASP)、舒张压(ADP)、平均动脉压(MAP)及冠脉灌注压(CPP)水平变化,同时观察其抢救成功率。结果患者治疗后ASP、ADP、MAP、CPP水平较治疗前明显升高,差异有统计学意义(P<0.05);其中抢救成功54例,抢救成功率为90.00%(54/60)。结论在心肺复苏治疗中,静脉推注肾上腺素药物的作用机制是受α_1受体激动剂作用,通过增强外周血管阻力,提高CPP,恢复心肌供血,有利于提高其抢救成功率。
Objective To investigate the clinical effect and mechanism of epinephrine in cardiopulmonary resuscitation. Methods June 2016 - February 2017 Sixty patients with cardioversion undergoing cardiopulmonary resuscitation in the Second People’s Hospital of Guiyang were enrolled in this study. Intravenous injection of epinephrine 1 mg was followed by intravenous injection of epinephrine 1 mg + saline 10 ml After cardiopulmonary resuscitation, they were immediately sent to ICU ward to monitor changes of aortic systolic pressure (ASP), diastolic pressure (ADP), mean arterial pressure (MAP) and coronary artery pressure (CPP) in all patients before and after using epinephrine , While observing the success rate of rescue. Results The levels of ASP, ADP, MAP and CPP in patients after treatment were significantly higher than those before treatment (P <0.05). Among them, 54 were successfully salvaged and the success rate of salvage was 90.00% (54/60). Conclusion The mechanism of intravenous injection of adrenergic drugs during cardiopulmonary resuscitation (CPR) is mediated by α 1 receptor agonist, which can improve the success rate of rescue by increasing peripheral vascular resistance, increasing CPP and restoring myocardial blood supply.
