目的探讨重症基层骨干医师短期培训方式及效果。方法选取2010年10月-2014年10月新疆医科大学第一附属医院行短期培训的68名重症基层骨干医师,采用随机数字表法分为两组,实施问题式学习(problem-based learning,PBL)教学模式重症基层骨干医师为对照组,实施案例式学习(case-based learning,CBL)教学模式重症基层骨干医师为观察组,各34名,比较两组重症基层骨干医师的培训方式及效果。结果观察组问卷调查得分(认可度、趣味性、知识点理解力、重症疾病治疗原则把握度、总评分)、考核成绩(理论考核、临床操作、病例分析)、评判性思维能力评分(开放思想、分析能力、系统化能力、评判思维自信心、寻找真相、求知欲、认知成熟度、总评分)、重症基层骨干医师对培训模式的满意度(知识点涵盖、提高学习能力、增加学习兴趣、提高临床技能)均明显高于对照组,差异均有统计学意义(P<0.05)。结论 PBL、CBL均是重症基层骨干医师短期培训的常用方法,其中CBL的培训效果更佳,不仅能提高医师技能,还能提高医师满意度。
Objective To explore the short-term training methods and effects of physicians in severe primary care. Methods A total of 68 critically ill key cadres from the First Affiliated Hospital of Xinjiang Medical University from October 2010 to October 2014 were selected and divided into two groups according to the random number table. The CBL teaching mode was implemented in 34 primary cadres at the grassroots level. The training methods and effects were compared between the two groups. Results The score of questionnaire survey in the observation group (recognition degree, interest, understanding of knowledge points, grasping degree of the critical illness treatment principle, total score), test scores (theoretical assessment, clinical operation, case analysis), score of critical thinking ability , Analysis ability, systematic ability, self-confidence in judgment and thinking, finding the truth, curiosity, cognitive maturity and total score), satisfaction of training mode of critical physicians (covering knowledge points, improving learning ability and increasing interest in learning , Improve clinical skills) were significantly higher than the control group, the difference was statistically significant (P <0.05). Conclusions Both PBL and CBL are common methods for short-term training of critically ill key cadres. Among them, CBL training is more effective and can not only improve the physician skills, but also improve the physician’s satisfaction.
