目的应用 CT 灌注的定量方法对不同级别胶质瘤的脑血容量(CBV)和血管通透性(Ps)进行相关性分析,探讨两者在评估胶质瘤术前分级中的作用。方法对19例经手术病理证实的胶质瘤患者进行 CT 血流灌注成像及常规 MR 增强扫描,并根据病理分级将患者分为低、高级别两组进行分析。CT 原始图像经灌注软件处理获得时间-密度曲线及灌注参数。采用 SPSS 11.0软件对灌注参数进行 Kruskal-Wallis 检验及相关性统计学分析。结果低、高级别胶质瘤的 CBV 中位值分别为2.7、6.5 ml/100 g,PS 中位值分别为0.389、12.810 ml·100 g~(-1)·min~(-1),差异有统计学意义(t 值分别为12.907、13.500,P 值均<0.05)。低、高级别胶质瘤的 CBV 与 PS 间 Pearson 相关系数(r)值分别为-0.058、0.648,仅在高级别组内 CBV 和 PS 才存在相关性(P<0.05)。结论 PS 和 CBV 对胶质瘤分级均具明显的相关性,在低级别胶质瘤中 CBV 与 PS 无相关性。
Objective To evaluate the correlation between the cerebral blood volume (CBV) and vascular permeability (Ps) in different grades of gliomas by using CT perfusion quantitative method and to explore their roles in the evaluation of preoperative gliomas grading. Methods Twenty-nine patients with pathologically confirmed gliomas underwent CT perfusion imaging and routine MR enhancement scanning. The patients were divided into two groups according to their pathological grade. CT original images were processed by perfusion software to obtain time-density curve and perfusion parameters. Kruskal-Wallis test and correlation analysis of perfusion parameters using SPSS 11.0 software. Results The median CBV of low and high grade gliomas were 2.7 and 6.5 ml / 100 g, respectively, and the median PS values were 0.389 and 12.810 ml · 100 g -1 · min -1, respectively Statistically significant (t = 12.907, 13.500, P <0.05). Pearson’s correlation coefficient (r) between CBV and PS in low and high grade gliomas were -0.058 and 0.648, respectively. Correlation was found between CBV and PS only in the high-grade group (P <0.05). Conclusions Both PS and CBV have obvious correlation with glioma grade. There is no correlation between CBV and PS in low grade gliomas.
