目的:探讨幽门螺杆菌(Helicobacterpylori,Hp)的空泡形成毒素(vacuolatingcytotoxinA,VacA)、细胞毒素相关蛋白(cytotoxinassociatedproteinA,CagA)及尿素酶A(ureaseA,UreA)以及另外3种Hp特异蛋白(相对分子质量分别为19.5×103,26.5×103和35×103)与消化性溃疡(pepticulcer,PU)及慢性浅表性胃炎(chronicsuperficialgastritis,CSG)的关系,了解这6种蛋白在溃疡的存在及发生中的意义。方法:以山东牟平胃癌高发区人群为调查样本,以WesternBlot的方法,检测病人血清中上述6种Hp特异蛋白的抗体,并以LEICAQ550IW软件系统对抗体表达进行定量测定。同时对胃镜下活检组织标本进行HE染色,以分析组织的病理学特征。全部标本的Hp诊断以WS染色及快速尿素酶试验(RUT)均阳性为金标准。结果:(1)PU组与CSG组中各蛋白抗体的检出率无显著差异(P>0.05)。而定量研究发现,①CSG组26.5×103蛋白的含量高于PU组[(0.057±0.043)vs(十二指肠球部溃疡:0.040±0.032),(胃溃疡:0.023±0.017),P<0.01];②17例CSG病人在一年后发展为十二指肠球部溃疡,他们1996年血清中35×103蛋白抗体的含量高于同期与之配对的17例未发生十二指肠球部溃疡的CSG病人[(0.107±0.026)vs(0.085±0.036),P<0.01]。但胃溃疡组无此差别。(2)26.5×103蛋白的抗体
Objective: To investigate the expression of vacuolating cytotoxin A (VacA), cytotoxinassociated protein A (CagA) and urease A (UreA) in Helicobacter pylori (Hp) and three other Hp specific proteins (19.5 × 103, 26.5 × 103 and 35 × 103), peptic ulcer (PU) and chronic superficial gastritis (CSG), and to understand the relationship between these six proteins in the presence and occurrence of ulcer Meaning.
