目的分析超重肥胖儿童代谢综合征(MS)各组分的聚集特征,探索儿童代谢综合征的病理生理机制。方法研究对象来自前期工作收集的2批样本,为北京市835名14,15岁超重肥胖儿童,其中样本1有93人(超重62人,肥胖31人)、样本2有742人(超重371人,肥胖371人)。先对样本1中超重和肥胖儿童代谢综合征的组分(包括体质指数、腰围、血糖、血脂、血压)进行因子分析,后对样本2进行因子分析用以验证和确认分析结果。结果超重儿童MS组分共提取4个因子,脂代谢因子(LDL-C,TC,TG,HDL-C)、血压因子(SBP,DBP)、肥胖因子(WC,BM I)和糖代谢因子(FPG,HDL-C),累积方差贡献率达到77.44%;肥胖儿童MS组分共提取3个因子,肥胖-血压因子(WC,BM I,SBP,DBP)、脂代谢因子(LDL-C,TC,TG,HDL-C)和糖代谢因子(FPG),累积方差贡献率达到69.84%。结论儿童MS的病理生理机制复杂,任何一个因子都不能单独解释MS的全部变量。
Objective To analyze the aggregation characteristics of various components of metabolic syndrome (MS) in overweight and obesity children and to explore the pathophysiological mechanism of metabolic syndrome in children. Methods The subjects were from two batches of samples collected from the previous work, including 835 overweight and obese children aged 14 and 15 in Beijing, including 93 in sample 1 (62 overweight and 31 obese), 742 in sample 2 (371 overweight , 371 obese). The first component of the metabolic syndrome in overweight and obese children in sample 1 (including body mass index, waist circumference, blood glucose, blood lipids, blood pressure) were analyzed by factor analysis and sample 2 was used to verify and confirm the analysis results. Results Four factors were extracted from the MS group of overweight children. The levels of lipid metabolism (LDL-C, TC, TG, HDL-C), blood pressure (SBP, DBP), obesity (WC, BM I) and glucose metabolism FPG and HDL-C). The cumulative variance contribution rate reached 77.44%. Three components of obesity children were extracted from MS component, including obesity-blood pressure factor (WC, BMI, SBP, DBP), lipid metabolism factor , TG, HDL-C) and glucose metabolism factor (FPG), the cumulative variance contribution rate reached 69.84%. Conclusion The pathophysiology of childhood MS is complicated. No single factor can explain all the variables of MS independently.
