Application of healthy donor sperm for artificial insemination has been widely used for male infertility factors. Until January 1986, Cleveland Clinical Foundation ruled out using fresh semen in its donor insemination program and created a cryopreserved, non-immunocompromised (cryptic) vaccine in 1985 due to increased transmission of infectious agents, particularly human immunodeficiency virus (HIV) With the understanding of donor practices, pathogen sperm banks, prior to spending too much time and money, have established screening programs to eliminate unqualified and unfettered applicants. Although the criteria for screening prospective donors have been recommended, the frequency and cause of exclusion of donors has not been well described. This study is undertaken to determine the cause of donor being excluded as well as semen
