目的:探讨人血清鸢尾素(Irisin)和分泌型卷曲相关蛋白5(secretory curl related protein 5,SFRP5)水平变化与2型糖尿病合并肥胖患者的关系。方法:选取2017年7月至2018年10月在孝感市中心医院内分泌科接受治疗的2型糖尿病合并肥胖患者38例作为观察组,再取同期健康体检者44例作为对照组。比较两组患者体重指数(body mass index,BMI)、空腹血糖(fasting plasma glucose,FBG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白-胆固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白-胆固醇(low density lipoprotein-cholesterol,LDL-C)水平,血清鸢尾素(Irisin)、分泌型卷曲相关蛋白5(secretory curl related protein 5,SFRP5)水平,并分析相关指标与血清Irisin、SFRP5水平的相关性。结果:观察组患者BMI、FPG、HbA1c水平均显著高于对照组(n t值分别为28.71、16.74、13.87,n P值均<0.05),HDL-C水平均显著低于对照组(n t=4.640,n P<0.05)。观察组患者血清Irisin、SFRP5水平显著低于对照组(n t值分别为15.80、7.152,n P值均<0.05)。观察组患者BMI、FPG、HbA1c与血清Irisin水平均呈显著负相关(n r值分别为-0.803、-0.336、-0.644,n P值均<0.05),HDL-C与血清Irisin水平均呈显著正相关(n r=0.845,n P<0.05);观察组患者BMI、FPG、HbA1c与血清SFRP5水平均呈显著负相关(n r值分别为-0.745、-0.627、-0.614,n P值均<0.05),HDL-C与血清SFRP5水平均呈显著正相关(n r=0.711,n P<0.05)。n 结论:血清Irisin、SFRP5的水平越低,2型糖尿病合并肥胖患者的病情越严重,故其对2型糖尿病合并肥胖患者的病情诊断有重要的临床意义。“,”Objective:To explore the relationship between the change of human serum Irisin & secretory curl related protein 5 (SFRP5) and type II diabetic patients with obesity.Methods:Total of 38 patients with type 2 diabetes mellitus and obesity who were treated in the Endocrinology Department of Xiaogan Central Hospital were selected as the observation group, and 44 healthy people in the same period were selected as the control group from July 2017 to October 2018.The body mass index (BMI), fasting plasma glucose (FBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol (LDL-C) andserum Irisin and SFRP5 level were compared between the two groups.The correlation between the related indexes and serum irisin, Sfrp5 levels has also been analyzed.Results:The levels of BMI, FPG and HbA1c in the observation group were significantly higher than those in the control group (n t values were 28.71, 16.74 and 13.87 respectively, all n P values < 0.05), and HDL-C were significantly lower than those in the control group ( n t= 4.640, n P<0.05). The serum irisin and Sfrp5 levels in the observation group were significantly lower than those in the control group (n t values were 15.80 and 7.152 respectively, both n P values<0.05). The BMI, FPG and HbA1c of the patients were significantly negatively correlated with the serum irisin level in the observation group (n r values were -0.803, -0.336 and-0.644 respectively, all n P values <0.05), and a significant positive correlation between HDL-C and serum irisin has also been found in the group( n r= 0.845, n P<0.05). The BMI, FPG, HbA1c and serum Sfrp5 levels were significantly negatively correlated in the observation group (n r values were -0.745, -0.627 and -0.614 respectively, all n P values<0.05), and a significant positive correlation between HDL-C and Sfrp 5 has been found in the group (n r= 0.711, n P<0.05).n Conclusion:The lower the level of serum irisin and Sfrp5, the more serious the condition of type 2 diabetes patients with obesity, so it has important clinical significance for the diagnosis of type 2 diabetes patients with obesity.
