目的观察克氏针钢丝张力带内固定术用于治疗儿童肱骨髁上骨折的临床疗效。方法回顾性分析2014年1月至2015年6月期间收治的66例肱骨髁上骨折患儿的临床资料,患儿按照手术方式进行分组:30例行肘内外侧联合入路小切口克氏针钢丝张力带内固定术治疗为观察组,36例行传统肘正中切口行克氏针交叉内固定治疗为对照组,观察和比较两组的手术效果。结果两组手术时间无明显差异[(65.32±9.78)min vs(62.59±8.87)min,P>0.05],观察组的术中出血量[(55.25±8.3)ml vs(63.19±10.69)ml]、术后住院时间[(6.45±1.52)d vs(7.83±1.94)d]及骨折愈合时间[(56.21±5.84)d vs(71.16±10.32)d]均较对照组显著减少(P均<0.01);观察组肘关节功能优良率为96.67%,显著高于对照组的75.00%(P<0.05);观察组术后未见并发症,与对照组的19.44%比较差异显著(P<0.05)。结论克氏针钢丝张力带内固定术用于治疗儿童肱骨髁上骨折具有操作简便、内固定牢靠及预防肘内翻畸形等效果。
Objective To observe the clinical effect of Kirschner wire internal fixation with tension band in the treatment of supracondylar fracture of humerus in children. Methods The clinical data of 66 children with supracondylar fracture of humerus who were treated between January 2014 and June 2015 were retrospectively analyzed. The children were divided according to the surgical method. Thirty patients with small incision Kirschner wire Tension-band internal fixation was used in the observation group. Twenty-six patients underwent traditional Kirschner wire internal fixation with the median incision as the control group. The surgical results were observed and compared between the two groups. Results There was no significant difference in operative time between the two groups [(65.32 ± 9.78) min vs (62.59 ± 8.87) min, P> 0.05]. The intraoperative blood loss in the observation group was (55.25 ± 8.3) ml vs (63.19 ± 10.69) ml (P <0.01), postoperative hospital stay [(6.45 ± 1.52) d vs (7.83 ± 1.94) d] and fracture healing time (56.21 ± 5.84 d vs (71.16 ± 10.32) d vs ). The excellent and good rate of elbow joint function in the observation group was 96.67%, which was significantly higher than that in the control group (75.00%, P <0.05). No complications were observed in the observation group, which was significantly different from the control group (19.44% . Conclusion Kirschner wire tension band internal fixation for the treatment of children with supracondylar humerus fractures has the advantages of easy operation, solid fixation and prevention of cubitus varus and other effects.
