为探讨慢性阻塞性肺病(COPD)患者外周及心腔内血浆心房利钠肽(hANP)浓度的变化及其与肺循环血流动力学的关系,我们对一组COPD患者进行了有心导管检查,测定不同心腔压力及相应各部位和外周血浆hANP浓度。结果发现,COPD组患者外周及不同心腔部位的血浆hANP浓度明显高于正常对照组(P<0.05),且与平均肺动脉压之间呈正相关,而与肺毛细血管楔压、心脏指数和平均右房压无相关关系。COPD患者血浆hANP浓度升高与有心血流动力学异常有关,可能是对肺动脉高压的一种代偿。
To investigate the changes of peripheral and intra-atrial plasma atrial natriuretic peptide (hANP) concentrations in patients with chronic obstructive pulmonary disease (COPD) and their relationship with pulmonary hemodynamics, we performed a cardiac catheterization in a group of COPD patients, Different intracardiac pressure and corresponding parts and peripheral plasma hANP concentration. The results showed that the concentration of plasma hANP in peripheral and different cardiac chambers of patients with COPD was significantly higher than that of normal controls (P <0.05), and positively correlated with mean pulmonary arterial pressure, but not with pulmonary capillary wedge pressure, cardiac index and mean Right atrial pressure has no correlation. COPD patients with elevated plasma concentrations of hANP and cardiac hemodynamics abnormalities may be a compensatory pulmonary hypertension.
